The family registry system is a record of births, deaths, marriages and other information. In accordance of the family registration law, foreign national residents must notify city hall or town hall of any births, deaths, and marriages.

2-1 Birth registration

(1) Apply within 14 days after the birth (within 3 months after the birth, if a child of Japanese spouse was born outside of Japan).

(2) Applicant shall be either father or mother.

(3) Applicant shall apply to birthplace, present address of applicant, or permanent address of spouse of Japanese national.

(4) Required items to be submitted are:
  • (A) Birth Registration Form with Birth certificate (with Japanese translation , if& the certificate is in foreign language)
  • (B) Passport of parents, if they are foreign nationals
  • (C) Your resident card or special permanent resident ceritificate
  • (D) Maternal and child health handbook
  • (E) Seal of applicant (if applicant is Japanese national)

2-2 Notification of death

Notification of death shall be made as follows

(1) Apply within 7 days after the person was confirmed to be dead.

(2) Notifier: Relatives, housemates, landlords where the deceased died, landowners or house or land managers, guardians, assistants, or voluntary guardians

(3) Notification: The address of the reporter, the registered domicile of the Japanese spouse, or the city, ward, town or village of the place of death.

(4) Required items to be submitted are:
  • (A) Death Registration Form with Death certificate
  • (B) Residence card, Special Permanent Resident Certificate (reporter)

2-3 Marriage registration

The following items are required for marriage registration

(1) Marriage registration form (signature of two adult witnesses is required in addition to the signature of both parties)

(2) Certificate of marriage requirements (issued by a foreign spouse’s home government, local government or other authorized person, such as foreign diplomatic offices in Japan) (with (signature of two adult witnesses is required in addition to the signature of both parties) Japanese translation)

(3) Birth certificate (with Japanese translation)

(4) Passport with Japanese translation attached

(5) The resident card or the special permanent resident certificate

(6) Identification document (driver's license, passport, etc.)

(7) Other documents as required

2-4 Divorce registration

The following items are required for divorce registration.

(1) Divorce registration form (signature of two adult witnesses is required in addition to the signature of both parties)

(2) Passport (with Japanese translation)

(3) A certified copy of resident registration of Japanese spouse (a certified copy of foreign spouse listing status of residence and period of stay, if Japanese spouse is outside of Japan )

(4) Identification document (driver's license, passport, etc. )

2-5 Naturalization

To acquire Japanese nationality, you must obtain permission from the Minister of Justice. Then you must notify the city hall.
The folllowing documents are required to naturalization registration.

(1)Naturalization registration form (if you have a Japanese spouse, his/her signature .)

(2) Your identification card ( issued by District Legal Affairs Bureau)

(3) Family registry of Japanese spouse (if he or she registers in his or her legal domicile, it is not required).
